2011-2012 Hockey Season!

REGISTRATION IS CLOSED!

General registration information

You can register for Edgcumbe Hockey on-line, in-person, or by US mail.

Online registration will be incomplete, as we need a signature for one document and we don’t have an online payment method.  You’ll still need to bring this document and payment to in-person registration or mail them.

For in-person and US mail registration, you’ll still need to complete the USA Hockey registration online.

What you need to do:

Register with USA Hockey and obtain a USA Hockey confirmation page [see below for more information]

Complete the Edgcumbe Hockey registration form [see below for more information]

Print and Sign the Consent to Treat Form [see below for more information]

Provide a copy of birth certificate, if new player [more information]

Write a check payable to Edgcumbe Hockey

You’ll need to bring to in-person registration or US mail:

            The signed Consent to Treat form

Register with USA Hockey and obtain a USA Hockey confirmation page

Go to the USA Hockey Registration website.

Choose the member type “Ice Players & Coaches”.

Follow the instructions to complete the USA Hockey registration.

You will receive a confirmation page that confirms you are registered with USA Hockey and Minnesota Hockey.

Email the confirmation page to Steve Anderson, bring a copy of the confirmation page to in-person registration, or US mail the confirmation page.

No Waiver of Liability form is required. When you complete online registration, you also complete the USA Hockey Waiver of Liability, Release Assumption of Risk & Indemnity Agreement.  Otherwise, we’re required to obtain a completed Waiver of Liability form.

Complete the Consent to Treat form

We’re required to obtain a completed Consent to Treat form.  We need a signature on this form, so you’ll need to bring it to in-person registration or send via US mail. 

Provide a copy of birth certificate, if you’re new to Edgcumbe Hockey

We need a birth certificate as verification of the player’s age.  A birth certificate or passport is acceptable.  A baptismal certificate is not.  The birth certificate does not need to be a certified copy, a photocopy is fine.
